Setting a category specifically saves this information in the tags, and it can be overwritten when the file is downloaded again. More reliable way would be to use the file type folder, instead of setting the file type manually, you can find more info about it here...

Yes, this is how it's supposed to work now. It only shows the Intro mark for next track, and no markup for the played track (it's played anyway so it doesn't make much sense there).

Yes, this is by design - when the relay is active, it will send the titles from the relay.

I don't know how it's possible, if the logs show that the song had started, then it had started; if it played depends on what happens next, e.g., a scheduled event that starts next, can make the song skip. You should check the Logs to see what happens. Sample rate does not affect anything...
